• Anudaan form with other required documents to be submitted by hospital authorities where the transplant is planned.
• Funds will only be disbursed to the hospital, and not to the family, after the submission of the final discharge bill
• Applications must be made at least 15 days prior to the transplant date
• The final decision on the applications (based on eligibility criteria) will depend on the discretion of the internal committee set up for this purpose.
